As school winds down, just wanted to remind you of the dreaded yet required medical paperwork that needs to be completed and returned prior to the start of the school year.  All medication, physical forms and care plans need to be filled out completely each year by your child’s health care provider.  A parent and health care provider signature is also required on these forms.
For Incoming Kindergarten Students;

Copy of 5 year old physical and additional required vaccines.  Usually includes 2nd MMR and additional doses of both the IPV and DTaP vaccines since Pre-K enrollment.
For Upcoming Sixth Grade Students:

Copy of Tdap and meningococcal vaccines.  Copy of 11 year old physical not required but appreciated.
For Fifth to Eighth Grade Sport Participants (Cheerleading and Basketball):


Annual Athletic Pre-Participation pages 1, 3 and 4 completed and signed by both parents and health care provider.  Date of Physical Exam must be within one year of start of tryouts.  If physical performed more than 90 days prior to tryouts, students must also submit “Health Update Form” completed and signed by parent.  All of these forms are below, on our website under parent resources or from the nurse or main office.
